BACKGROUND INFORMATION
Carolyn Puckett, a Licensed Clinicl Social Worker has been in private
practice since 1992. Extensive experience working with children,
adolescents, families, couples and individuals.
on both an outpatient and inpatient basis. A graduate of Louisiana State
University with a masters degree in Social Work.
PSYCHOTHERAPY
Working from a Christ-centered, biblically based model using
cognitive-behavioral, insight oriented approach Ms. Puckett is able to help
individuals, couples and families.
ADOPTION HOMESTUDY
Ms. Puckett works with couples who desire to adopt a child. Part of the
adoption process is the completion of an extensive homestudy in which she
is able to meet with the individual in their home and in her office for a
complete assessment. The assessment includes a criminal history background
check; reference letters from family and friends and letters from each
individuals primary care physican and/or Gynecologist. Upon completion of
the assessment a comprehensive report is written and submitted to the
agency with whom the couple is working.
PRE-ADOPTION COUNSELING
Pre-adoption counseling is provided to the birth parent who is
relinquishing custody of her child.
Counseling as mandated by law for birth mothers and to assist them in the
transition to relinquish custody of their child upon birth.
POST ADOPTION HOME VISITS
Supervision of adoptive parents and their adopted child. As required by Louisiana Administrative Code supervision accomplished in the home followed by an evaluation to the appropriate agency.

Ms. Puckett is a Board Member of Christian Oasis a ministry for hurting
people located 2801 Donahue Ferry Road, Pineville, Louisiana 71360,
telephone 640-1566. She works very closely with Jonathan Forest, M.D.
consulting with him as well as working with him to minister to hurting
people. Member of the National Association of Social Workers.
OTHER:
EDUCATION/GROUP THERAPY
Beginning October 8, 2003 an education workshop and
group therapy for individuals suffering with chronic pain associated with
Fibromyalgia, Chronic fatigue Syndrome and other disabling illnesses. Each
Wednesday Ms. Puckett will meet with individuals from 11:00 a.m. till 12:45
p.m. Cost : $20.00 per week.
